How All-on-4 Works
Four implants are placed in the jawbone, typically with the two rear implants angled to take advantage of available bone and avoid the sinus cavity in the upper jaw. A full-arch prosthesis — a complete set of teeth — is then attached to the four implants. The angled placement strategy means that most patients qualify for the procedure even with moderate bone loss, without needing bone grafts first.
All-on-X follows the same principle but uses additional implants, typically five or six, for patients with more complex anatomy or greater bone availability. The added implants distribute load more evenly and can support a slightly heavier prosthesis.
Who Is a Candidate
All-on-4 is designed for patients who are missing all teeth in an arch, or who have failing teeth that need to be removed. Patients must have adequate bone volume at the implant sites, be in reasonably good overall health, and be non-smokers or willing to stop smoking before and during the healing period. A cone beam CT scan taken at our office gives us the three-dimensional view needed to assess bone volume and plan implant positioning precisely.
The Procedure
Failing teeth are extracted if necessary, the four or more implants are placed in a single surgical session, and a temporary fixed prosthesis is attached the same day. Patients leave with a full set of functional teeth. The implants integrate with the jawbone over three to six months, after which the final permanent prosthesis is fabricated and attached.
All-on-4 vs. Traditional Dentures
Traditional dentures rest on the gum tissue and must be removed daily. They shift during eating and speaking, require adhesive, and over time accelerate bone resorption because they do not stimulate the underlying jaw.
